Actinomycosis is an unfrequent bacterian infection, that affects 50% of the cases in the cervical facial region. The evolution and clinical, anatomopathologic radiologic characteristic, and its presentation bring us to suspect another type of more frequent pathology: the neoplastic, that make very difficult its diagnosis. We present the case of a young patient affected by a cervical tumoration initially oriented as Hodgkin lymphoma that finally was diagnosed as cervical actinomycosis and treated with penicillin.

La actinomicosis es una infección bacteriana crónica poco frecuente que afecta en un 50% de los casos a la región cervicofacial. Tanto su evolución como las características clínicas, anatomopatológicas y radiológicas con que se presenta nos llevan a sospechar otro tipo de patología más frecuente como la neoplásica por lo que su diagnótico suele ser difícil. Presentamos el caso de una paciente joven con una tumoración cervical orientada inicialmente como linfoma de Hodkin que fue diagnosticada finalmente de actinomicosis cervical tratada con penicilina

Wegener granulomatose is an granulomatous necrotisant vasculitis of unknown ethiology that can affect in a classical form the superior and inferior respiratory tract and kidney. The possibility that the illness is limited to the ENT sphere together to a good accessibility for the biopsy of the nasal fossae and sinus usually convert the ENT in the first specialist for the diagnosis of the illness. It is a systemic illness with a frequent and important ENT expression. We have he possibility to establish an earlier diagnosis with prognostic implications in those patients with ENT clinic. So these patients can benefit the treatment with Cotrimoxazol. We have done a revision of the patients affected of Wegener granulomatose with ENT clinic, treated in the last ten years in our Hospital. We present 9 cases, exposing the age of diagnosis, the initial data, the topographic extension along the illness, the diagnostic methods and the treatment and evolution.

The parapharyngeal space is considered the key space of the suprahioid neck, being essential to establish the origin and anatomic relations of the lesions of the profound neck. The descriptions of the fascial limits of the parapharyngeal space are variable in the litterature, reflecting some ambiguity in the manuals of anatomy and surgery. We have done a detailed study of this region, working on 5 cadavers, fixed in a solution of phenol and alcohol, and correlating the disection pieces with a radiologic study. The goal has been to improve our anatomic knowledge of the suprahioid neck, to be able to establish an anatomo-clinic-radiologic correlation of the lesions of this region, to be able to establish an differential diagnosis, predict possible ways of dissemination and facilitate the most proper surgical approach.

El espacio parafaríngeo se considera el espacio clave del cuello suprahioideo, siendo esencial para establecer el origen y relaciones anatómicas de las lesiones que asientan en el cuello profundo. Las descripciones de los límites fasciales del espacio parafaríngeo son variables en la literatura, reflejándose cierta ambigüedad en los tratados de anatomía y de cirugía. Hemos realizado un estudio detallado de dicha región trabajando sobre 5 cabezas humanas fijadas en solución de fenol y alcohol, y correlacionando las piezas de disección con un estudio radiológico. El objetivo ha sido aumentar nuestro conocimiento anatómico del cuello suprahioideo para así poder establecer una correlación anátomo-clínico-radiológica de las lesiones que asientan en esta región y poder establecer un diagnóstico diferencial, predecir posibles rutas de diseminación y facilitar su abordaje quirúrgico más adecuado (AU)

La granulomatosis de Wegener es una vasculitis granulomatosa necrotizante de etiología desconocida que puede afectar de forma clásica al tracto respiratorio superior e inferior y a los riñones. La posibilidad de que la enfermedad se encuentre limitada a la esfera ORL junto a la buena accesibilidad para biopsia de las fosas nasales y los senos convierten habitualmente al otorrinolaringólogo en el especialista de primera línea en el diagnóstico de esta enfermedad. Se trata de una enfermedad sistémica con una frecuente e importante expresión otorrinolaringológica. Se tiene la posibilidad de establecer un diagnóstico precoz con implicaciones pronósticas en aquellos pacientes con clínica otorrinolaringológica. Además estos pacientes deben beneficiarse del tratamiento con Cotrimoxazol. Hemos realizado una revisión de los pacientes afectos de granulomatosis de Wegener con clínica otorrinolaringológica tratados en los últimos 10 años en nuestro hospital. Se presentan nueve casos clínicos, exponiendo de ellos la edad diagnóstica, la clínica de debut, la extensión topográfica a lo largo de la enfermedad, los métodos diagnósticos y el tratamiento y evolución (AU)

The fibrous dysplagia is a rare benign osseous pathology of unknown aetiology. It presents an incidence of craniofacial affectation of a 10% in its monostotic variety and a 100% in the poliostotic. The temporal bone implication is very rare, being usually referred in the monostotic variety. The diagnosis is based on radiology (CT) and histology. The majority of the authors agree in a conservative treatment. We present three clinical cases of monostotic fibrous dysplagia with craniofacial affectation, two of them in the temporal bone and another one in the frontal bone.

In the preantibiotic era laryngeal tuberculosis was the most frequent pathology of the larynx. In the last decades some changes in the presentation and evolution of the pathology have occurred due to the new chemiotherapy and the improvement of the nutrition. Actually it presents less incidence and infectivity and less evidence of pulmonary disease. In this article we analyse, on occasion of a clinical case and with a revision of the literature, the actual patterns of the laryngeal tuberculosis.

La displasia fibrosa es una enfermedad benigna ósea infrecuente, de etiología desconocida. Presenta una incidencia de afectación craneofacial del 10 por ciento en su variedad monostótica y del 100 por ciento en la poliostótica. La implicación del hueso temporal resulta poco frecuente, siendo principalmente documentada en la forma monostótica. El diagnóstico se basa en la radiología (TC) y la histología. La mayoría de autores están de acuerdo en seguir un tratamiento conservador. Nosotros presentamos tres casos clínicos de displasia fibrosa monostótica con afectación craneofacial, dos de ellos a nivel del hueso temporal y otro a nivel frontal (AU)

Cell Granular Tumor (CGT) are unusual tumors that can affect any site of the body, with most cases affecting the head and neck region. CGT of the larynx is an uncommon laryngeal tumor accounting for approximately 6-10% of the reported cases. In the larynx pseudoepitheliomatous hyperplasia may predispose to confuse the CGT with frothy (squamous) cell carcinoma. The origin of this tumor is a matter of debate but most authors believe it to be neural in origin.

The advance of computed tomography (CT) has made possible a more exact diagnosis of these infections. If CT suggests a cellulitis medical treatment is used, but abscesses need surgical drainnage.

Los tumores de células granulares (TCG) son tumores raros que pueden aparecer en cualquier parte del cuerpo con clara predilección por la región de cabeza y cuello. El TCG de laringe es un inusual tumor laríngeo, que constituye aproximadamente un 6-10 por ciento de los casos descritos. Squamous carcinoma of the external ear canal (CEEC) is an uncommon neoplasm which prognostic changes following the tumour spread. Because of its location in ECC as well as the anatomical fissures between osseous and cartilaginous parties it's possible the spread to areas of surgical accessibility complicated restricting the complete tumoral removal. We report one of those cases, when first visited had already meningeal infiltration and was accordingly treated with radiotherapy. Analysis of the case and review of the literature.

La miastenia gravis se trata de una enfermedad de inicio insidioso cuya primera manifestación de forma frecuente es la disfagia. Esta manifestación pasa a menudo desapercibida, por lo que el paciente es diagnosticado posteriormente cuando la enfermedad está más avanzada y la clínica es más evidente. Presentamos el caso de una mujer de 25 años de edad, que acudió a nuestro servicio por epistaxis, refiriendo ya en la primera visita una disfagia progresiva con endoscopia digestiva, TEGD, así como laringoscopia indirecta normales. El cuadro fue evolucionando hasta aparecer debilidad de extremidades a partir de la cualfue establecida la sospecha de miastenia gravis y posteriormente confirmada mediante electromiografía y la determinación de los anticuerpos antirreceptor de acetilcolina. Realizamos una revisión de la literatura sobre las manifestaciones clínicas, procedimientos diagnósticos y tratamiento de esta entidad. Remarcando la importancia de la sospecha de miastenia gravis en mujeres jóvenes con disfagia y antecedentes familiares o personales de enfermedades autoinmunes cara a un diagnóstico y tratamiento precoz (AU)
